Notice of Privacy Practices
The SLP Clinic at Widener University maintains records about client health as related to their speech, language, hearing and/or swallowing abilities. These records describe a client’s health history, symptoms, examination and test results, diagnoses, and any plans for care or treatment.
Clients of the SLP Clinic are asked to review our Notice of Privacy Practices prior to submitting an intake packet, in order to learn important information regarding how client health information may be used or disclosed by the SLP Clinic at Widener University, as well as client rights regarding personal health information, including the right to access my own records, and the right to request restrictions as to how my health information is used or disclosed.

Release of Clinical Records
All clients are entitled to request a copy of clinical documentation and records at any time. Documentation may be forwarded to other professionals at the client and/or parent/caregiver’s request. In addition, clinical supervisors and/or graduate clinicians may confer with other professionals outside the SLP Clinic only with written permission of the client and/or parent/caregiver via the Authorization to Release Clinical Records form. Careful records of these exchanges, whether written or oral, are maintained in the client’s chart.
