Client Handbook
In order to communicate critical information regarding client welfare and the nature of services at the SLP Clinic, clients are encouraged to read the client handbook prior to the first assessment and/or treatment session. The client handbook includes the following documents:
- Nondiscrimination Statement
- Clinical Services Consent
- Notice of Privacy Practices
- E-mail Consent Form
- Attendance Policy
- Clinic Rules
- Emergency Contact Form
- Medical Release Form
- Authorization to Release Clinical Records
These documents contain critical information pertaining to client welfare, including but not limited to: the nature of services within a university program, including that services will be provided by graduate students under the direct supervision of a licensed, certified SLP; the opportunity to opt out of video/audio recording of sessions as well as potential research opportunities; confidentiality and privacy practices; clinic policies; and the clinic and program’s commitment to diversity.
One signed copy of each document must be uploaded to the client’s chart. Once signed, these documents remain in effect until services are terminated. Graduate clinicians will collect signed documents review all information with clients during the first session, and can answer any questions at that time. Clinical sessions may not continue until these documents are signed. Per clinic policy, sessions will be canceled until forms are returned.
